For security reasons, My Lahey Chart is not able to generate an activation code via email or phone. You may obtain your activation code at any Lahey Health medical clinic. You may also visit the medical records department at any of the Lahey Health affiliated hospitals.

The most common reason for your My Lahey Chart to be deactivated is due to 5 failed login attempts. If this has occurred, please fill out the support form at the top of the page and we will reactivate your account within a few hours. 

UPDATE
EMAIL ADDRESS

Want to update your email address so you don't miss any messages related to My Lahey Chart? You can do this from within your account.

Follow the steps below to update your e-mail address.  <prototype example, below steps are not current state>

 

  • Log in to your My Lahey Chart account

  • Select "Preferences" on the left side of page

  • In the preference section, select "Change Email Address"

  • Type in your current email address

  • Enter your new email address twice for verification

  • Once you are finished, click "Save Changes".

PROXY ACCESS 

Keeping an eye on your families health is important.

 

To give someone access to your account or to be set up as a proxy to a family members account, you must complete a Proxy Consent Form and take it your physician’s office. 

 

Please note: Due to state and federal privacy laws, proxy access must be set up at your physicians's office with all parties present. All parties must be coherent and have a legal form of identity to set up proxy connections.

 

For the safety of your personal medical information, My Lahey Chart support cannot provide medical advice or answer medical questions. You are encouraged to contact your physician’s office for clinical questions.

 

You may contact your physician’s office via phone or from within your My Lahey Chart account by using the message centers “Ask a Medical Question” option.
